We currently collect and process the following information:

• Doctors Name & surgery address

• Current or former medical treatment


How we get your personal information and why we collect it


Most of the personal information we process is provided to us directly by you for one of the following reasons:


• To evaluate the needs of the individual

• To contact the individual either in person or remotely

• To ensure the individual’s health & safety needs are meet

We also receive personal information indirectly, from the following sources in the following scenarios:

• In some cases, a third party may refer an individual they are concerned about for us to contact directly.  We will obtain the individuals consent to proceed before any action is taken.

• We will retain the third parties name & contact details in case of emergency or if further contact is required


We use the information that you have given us in order to provide the highest quality of support and care during this crisis. 

We may share this information with our Friendship volunteers and professional counsellors working with us at this difficult time.  Your information will ONLY be shared with the person contacting you.  

Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for processing this information are:

(b) We need it to perform a public task.

(c) We have a legitimate interest.


How we store your personal information 


Your information is securely stored on a secure, encrypted server with limited, controlled access. On rare occasions we may need to print out an individual’s data, these will be secured in a locked, secure location unless in current use.


We keep all information for 12 months after the end of the crises in case of required follow up, statically purposes and reporting purposes. We will then dispose this information by removing all individual files from electronic files, saving and then deleting the electronic file.  Any hard copies of data retained will be shredded and disposed in a secure site for incineration.

Under data protection law, you have rights including:


Your right of access - You have the right to ask us for copies of your personal information. 

Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete. 

Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances. 

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances. 

Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.

Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.


You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
